▪︎ I Just Testified As an Expert Witness in This Case…(LNC)

Last month I testified as an expert witness in this unfortunate case involving a patient who was seen in the ED for severe, acute pancreatitis and severe dehydration. His condition was documented as “very unstable.”
 
Although one ED physician documented that the patient should be admitted to the ICU, when she went off shift another physician admitted the patient to the Telemetry Unit (on the night shift). The nurses on the floor did a good job of advocating for the patient to be admitted to the ICU, but nothing was done when they reported the situation to their House Supervisor.
 
Not surprisingly, the patient had significant electrolyte imbalances including a K of 6.7. Although this level was communicated to the admitting physician (and orders were obtained), several other critical lab levels were not, as well as some significant changes in condition that occurred later in the shift. During shift change the patient had an “unwitnessed cardiac arrest.” Although the patient was resuscitated, he was left with catastrophic anoxic brain injuries. It was later determined that the alarms on the cardiac monitor had been turned off.
